Depends on the flight. I found the changing table in the lavatory wasnt too bad, it folded down right over top the toilet. That was on a bigger plane. On the smaller plane I had to take for a connection we were all forced to stay seated due to turbulence so I changed him where he was. I found people just appreciated the fact he wasnt screaming his head off the entire flight so didnt mind a nappy change. I suggest during take off and landing you be prepared with a pacifier or bottle or even nurse him/her if necessary. It helps with equilizing pressure in the ear and prevents earaches. I also found it helpful to give a dose of tylenol 30 mins before the flight so if the ears didnt pop right it wasnt as painful. On a side note be careful of how low the shelf is above the changing table if in the lavatory. Be respectful of your other passengers. If you are going to do it in your seat, please ask them if they mind if you do it. If you are travelling with someone, take only the baby, the wipes and the diaper- it's not just tight, but damn you can barely turn around in there to get the door shut.
I changed my daughter's diaper at my seat on the flight to Germany after my first lavatory experience, no one said anything, probably because they too had a baby.
My flight back was better,we had a larger jet and the bathrooms were on the lower level and much larger so there was actually space.

If at all possible, wait until you land. I tried changing my 3 mo old on an empty seat next to me and it was a nightmare! Once you hit a layover or land, go do it right away. On Southwest airlines they didnt have a changing table in the bathroom so they gave me a blanket and i changed him on the floor, it wasn't that bad. People are usually very nice to people with babies and are generally very helpful.
